Step-by-Step Guide
Notify the DEA immediately
Contact the local DEA Diversion Field Office within 5 days of death. The DEA registration must be surrendered and cannot be transferred.
Secure controlled substances
All controlled substances in the practitioner's possession must be immediately secured. Do NOT dispose of them without DEA authorization.
Arrange proper disposal
The DEA will provide instructions for proper disposal or transfer of controlled substances. An authorized agent or another registrant must handle the process.
Return the DEA certificate
The original DEA Certificate of Registration must be returned to the DEA. Mail it to the DEA Registration Unit with a cover letter and death certificate.
