Step-by-Step Guide
Identify the HSA custodian
Contact the employer to determine which company holds the HSA (e.g., Optum, HealthEquity, Fidelity, Lively). The employer may contribute to the HSA, but the account belongs to the individual. Get the custodian's name and phone number.

Contact the HSA custodian
Call the HSA custodian to report the death. If the deceased named a beneficiary, the custodian will process the transfer. If the spouse is the beneficiary, the HSA transfers directly and continues as the spouse's own HSA with full tax advantages.
Submit outstanding medical expense claims
Before closing the account, submit reimbursement claims for any eligible medical expenses incurred by the deceased before death. These withdrawals are tax-free. Keep receipts for documentation.
Process the transfer or distribution
For spousal beneficiaries, complete the transfer paperwork to assume ownership. For non-spouse beneficiaries, the account will be distributed at fair market value and included as taxable income on the beneficiary's tax return.
